One of my fellow medical students - an excellent medical student! - took a year off for research prior to fourth year. I run into her on the elevator from time to time. What she told me, last time, was, I have a new respect for you MD/PhDs who did research.
I tried to reassure her that it usually takes a year or two to figure out what is going on, but by the third or fourth year, the research should come together. I don't think that was very reassuring.
There is a lot of wandering in the wilderness of unknowing before finding your way out.
